We are located on the lower level of the historic Eager Building, at 7 E Main Street, Suite 4, in Evansville, Wisconsin.

There is public parking both in front and back of the Eager Building. If you park in the back of the building, be aware the back door is always locked. You’ll need to call the front desk and we will come let you in. That number is 608-882-1388.

Yes! We have an elevator in the building and the entire building, including the bathroom in our clinic, is ADA accessible.
